Prepare for your meeting with Willis Law Group using a form tailored to your situation. Download a blank fillable PDF for estate planning, probate and estate administration, adult guardianship, or Medicaid planning. Official government applications and court resources are listed separately by state.
New inquiries: please contact our team first so we can confirm the matter, the relevant state, and the conflict-screening step. Complete a detailed intake when our office requests it. If you have a notice or deadline, identify the date promptly; do not wait to finish a questionnaire.
Download Your Intake Form
These are fact-gathering questionnaires, not court papers, Medicaid applications, or estate-planning documents. Each PDF asks which state is involved. The same preparation questionnaire can record facts about an NJ or NY situation; the applicable law, filing documents, and availability of representation require separate review.
Estate Planning Personal Information Form
Organize family and beneficiary information, your goals, possible decision-makers, existing documents, property ownership, income, debts, and planning questions. Approximate values and “unknown” answers are welcome. Read our estate-planning preparation checklist for help gathering documents.
Probate & Estate Administration Intake
Record the decedent’s residence and date of death, the location of a will or trust, family and beneficiary relationships, court status, assets, obligations, actions already taken, and questions for the attorney. Tell us about pending notices, disputes, or dates without deciding their legal effect yourself.
Adult Guardianship Intake
Describe the person’s current care and decision-making needs, family contacts, existing powers of attorney or other support, functional abilities, available evaluations, financial overview, and prior proceedings. This is an intake questionnaire; it does not determine whether guardianship is necessary or which court procedure applies.
Medicaid Planning & Application Intake
Gather care-setting details, applicant and spouse income, ownership and approximate asset values, prior transfers, existing authority, and application status. Include dates printed on requests for information, denials, renewals, or termination notices. Our New Jersey Medicaid-pending guide explains how to organize application follow-up records. For income-cap questions, read our NJ Qualified Income Trust and Miller Trust guide.
How to Fill, Save & Return Your Form
- Download the PDF and save it to a private device before typing. Use a PDF reader that supports fillable forms; if your browser does not save entries reliably, open the saved copy in a PDF reader.
- Save a completed copy and reopen it to check that your answers remain visible. You can also print the blank form and complete it by hand. Ask us for assistance if this format is difficult to use.
- Do not enter full Social Security numbers, full account numbers, passwords, or payment-card information. Provide additional sensitive records only through a return method our office has confirmed.
- Ask our team for a verified secure return method, or bring a printed copy to your meeting. Do not send completed sensitive questionnaires through the website’s general contact form or ordinary email. Filling in the PDF does not transmit it to our office.
No representation or deadline protection begins simply because a form is downloaded or completed. Our team must confirm the scope and engagement terms. These preparation forms are versioned October 29, 2026.
New Jersey Government Forms & Resources
Use these official sources for New Jersey matters, including Bergen County. Government applications serve a different purpose from the firm’s intake forms above. Confirm the correct application and submission route before filing.
- NJ FamilyCare Aged, Blind and Disabled (ABD): application options and supplemental forms — the state’s starting point for online and paper application information.
- Official NJ FamilyCare ABD application (PDF) and official document checklist (PDF).
- NJ MLTSS application guidance (2026 PDF) — long-term-services-and-supports application preparation.
- NJ Courts: establishing adult guardianship — official process guidance and links to applicable resources.
- Bergen County Surrogate: estate forms and resources — will and administration fact sheets; other counties may require their own materials.
New York Preparation Resources
Our New York practice is in development. These official resources help Rockland and Westchester families prepare. Contact our team to confirm whether we are accepting your particular New York matter before sending detailed information. NJ filing materials and Medicaid rules should not be used as substitutes for NY requirements.
The general fact-gathering intake downloads above include a state question and can help organize your records. They are not NY court petitions, an eligibility determination, or a commitment to accept a case.
- New York Department of Health: how to apply for Medicaid — application routes, including the Access NY application and Supplement A where applicable.
- New York Surrogate’s Court forms — official probate, administration, and other proceeding categories.
- New York CourtHelp: starting an Article 81 guardianship case — a separate resource from Surrogate’s Court guardianship forms; the appropriate proceeding depends on the situation.
Government links were reviewed October 29, 2026. Agencies and courts may update forms and instructions. Use their current requirements, and ask about any county-specific filing instructions.
