Care Resources: compare care types and browse counties
A family in Jersey City or Bayonne may be comparing a move into assisted living with support at home after a hospital stay. Start by separating the daily help needed from nursing or therapy needs, then ask each provider which services it can actually deliver. This Hudson County guide brings those research paths together.
Explore: Assisted living · Nursing homes and rehabilitation · Home care · County support · Care and legal planning.
These are selected research starting points, not a complete directory, ranking or endorsement. Listings and provider website descriptions were reviewed November 1, 2026. Confirm current services, licensing, availability, costs and payment policies directly; inclusion does not establish Medicaid acceptance or suitability for an individual.
Coordinate the discharge plan with the home setup
Before agreeing to a discharge destination, ask the care team for the expected assistance with transfers, medications and overnight supervision. For care at home, check stairs, bathroom access and whether the proposed visits leave gaps that family members cannot cover. For a facility, ask whether the admission is short-term rehabilitation or ongoing residence.
Assisted Living and Memory Care
- Oasis at the Atrium — Jersey City. Assisted living with personal-care support; its website also describes memory care. Match the current name and street address when checking state records.
Nursing Homes, Skilled Nursing and Rehabilitation
- Peace Care St. Ann’s — Jersey City. Long-term nursing care and short-term rehabilitation. Ask which clinical needs can be supported and how the proposed stay will be funded.
Home Health and Personal Care Companies
- BAYADA Hudson County Home Health — North Bergen team. Skilled nursing and therapy at home. Ask about a clinician’s order, service area and insurance authorization.
- Visiting Angels of Jersey City — Bayonne office; Jersey City and nearby communities. Personal care, companionship and respite services. Confirm the exact tasks, schedule and backup coverage.

You can also search the Aging Life Care Association directory for care-management professionals; ask about credentials, fees and referral relationships.
Check the Provider and Ask for Written Answers
Use New Jersey’s long-term care facility search to check the facility category and available inspection information. Use Medicare Care Compare for the relevant nursing-home or home-health provider type. For personal-care agencies, review New Jersey’s Health Care Service Firms resources.
- Care fit: Can the provider handle the assessed mobility, medication, dementia and overnight needs?
- Staffing and continuity: Who supervises care, responds after hours and covers absences?
- Total cost: Request base charges, additional care fees, deposits and termination terms.
- Payment changes: Ask what happens if care needs increase or private funds run low. Obtain the relevant policy in writing.
Care Costs, Medicaid and Authority to Act
Care selection and benefits planning are related but separate decisions. Medicare generally does not cover ongoing custodial long-term care; limited skilled or home-health coverage has its own conditions. A provider’s website does not determine eligibility or insurance authorization.
Connect care decisions with advance Medicaid planning, Medicaid crisis planning, or New Jersey Medicaid application representation. Existing powers of attorney and health care documents also deserve review when someone else will act.
Contact Willis Law Group to discuss the legal and financial questions in your care plan. Bring existing documents, the proposed care arrangement and any agency notices or deadlines. Ask our team how to provide confidential records. Clinical and placement decisions should involve the person, family and appropriate health professionals.
