Care Resources: compare care types and browse counties

Rockland County families often compare assisted living near Pearl River or Chestnut Ridge with nursing rehabilitation in New City and support at home. Use these selected New York resources to separate residential, clinical and personal-care options. A provider’s presence in this directory does not establish an available placement or benefits eligibility.

These are selected research starting points, not a complete directory, ranking or endorsement. Listings and provider website descriptions were reviewed November 1, 2026. Confirm current services, licensing, availability, costs and payment policies directly; inclusion does not establish Medicaid acceptance or suitability for an individual.

Match the New York program to the actual care setting

Ask an assisted-living provider which licensed or approved program would admit your relative and what needs it can accommodate. For nursing or home care, ask which assessment, referral and payer authorization are required. If relatives are coordinating from New Jersey, keep the New York resident’s care and benefits review separate from New Jersey rules.

Assisted Living and Memory Care

  • Promenade at Blue Hill — Pearl River. Residential assisted-living and memory-care research starting point. Confirm the program’s assessment and admission requirements.
  • Promenade at Chestnut Ridge — Chestnut Ridge. Assisted-living information from the operator. Ask which services and payment arrangements apply to the proposed admission.

Nursing Homes, Skilled Nursing and Rehabilitation

Home Health and Personal Care Companies

  • Sunshine Homecare Services — New City. Nursing and home-health-aide services. Confirm service availability, credentials and the duties included in the care plan.
  • VNS Westchester — Regional provider listing Rockland County in its service area. Home-health research starting point for Rockland residents. Confirm the referral, available disciplines and coverage at the home’s address.

County Support and Caregiver Resources

Rockland County Office for the Aging — County information about caregiver support, community services and care resources. You can also search the Aging Life Care Association directory for care-management professionals; ask about credentials, fees and referral relationships.

Check the Provider and Ask for Written Answers

Check New York’s adult-care facility profiles for Rockland County and Medicare Care Compare for the appropriate provider type. Match the facility’s legal name and address; do not apply a nursing-home rating to an assisted-living residence.

  • Care fit: Can the provider handle the assessed mobility, medication, dementia and overnight needs?
  • Staffing and continuity: Who supervises care, responds after hours and covers absences?
  • Total cost: Request base charges, additional care fees, deposits and termination terms.
  • Payment changes: Ask what happens if care needs increase or private funds run low. Obtain the relevant policy in writing.

Care Costs, Medicaid and Authority to Act

Care selection and benefits planning are related but separate decisions. Medicare generally does not cover ongoing custodial long-term care; limited skilled or home-health coverage has its own conditions. A provider’s website does not determine eligibility or insurance authorization.

For the financial and decision-making questions alongside care, explore our New York Medicaid planning guidance, New York powers of attorney and health care proxies, and Rockland County elder-law services. Explain where the person lives and receives care; New Jersey rules should not be assumed to govern a New York matter.

Contact Willis Law Group to discuss the legal and financial questions in your care plan. Bring existing documents, the proposed care arrangement and any agency notices or deadlines. Ask our team how to provide confidential records. Clinical and placement decisions should involve the person, family and appropriate health professionals.
